These documents will help us learn what we need to learn. Please be sure to complete these honestly and to the best of your ability. Bring them to your first session and let’s get started!
Need a few tools to help achieve your goal? Track your regular activities and see where you can use a boost! Use a Food Journal or Movement Journal as a tool to learn if you need to move more or eat less, or eat differently. These journals will go a long way toward helping you reach your fitness & health goals.
